 Learner Outcome: 

Nurses will be able to name the purpose of documentation and identify at least two strategies to protect their nursing license. 

A posttest will be available at the conclusion of the activity for those wishing to receive CNPD contact hours. A passing score of 80% is required to receive the certificate.

To receive contact hours for this activity, the learner will read the content, submit the evaluation, and pass the posttest with a score of 80% or better. The certificate will be awarded for 1.5 contact hours.

INTRODUCTION

Nursing documentation is both necessary and challenging. As vital healthcare team members, nurses continue to chart using various methods. Depending on your place of employment, you may still be using paper charting, but many facilities have made the switch to computerized charting. Some systems are more effective than others, but what does not change is the importance of what we document in medical records. A few documentation issues seem to be present continuously. Some of the issues may be time-related, and some may be due to facility policies, but some result from poorly developed habits. What you document in the patient’s medical record is a record of the care you give. It reflects your professionalism and ability to think critically. It shows that you know and understand the nursing process. This Nursing Continuing Professional Development activity is designed to identify some known problem areas and provide strategies to improve documentation positively.
